When you book your initial appointment, an online intake form will be sent to you. If you are unable to fill it out prior to your appointment, please arrive 5 minutes early to fill it out.
For your first appointment, the first 5-10 minutes will be used to review your health history and assess your health to create a treatment plan that is both safe and effective. Your health history must be updated annually or whenever a change in your health occurs.
